What Is LV Noncompaction?

Left ventricular noncompaction (LVNC) is a cardiomyopathy in which the myocardium of the left ventricle remains abnormally trabeculated and spongy instead of compacting fully during fetal development. The result is a thick, mesh-like inner muscle layer with deep recesses that trap blood. First described in detail in the 1990s, LVNC can occur in isolation or alongside other structural heart diseases. It is considered rare, though improved imaging has led to more frequent recognition.

How LVNC Develops

During normal embryonic heart development, the ventricular wall starts as a loose mesh of muscle fibers. By the end of the first trimester, compaction should compress this mesh into a solid, layered wall. In LVNC, that process arrests, leaving excessive trabeculations and a two-layered wall: a thin compacted outer layer and a thick, noncompacted inner layer. The exact trigger is not fully understood, but genetics appears to play a role. Mutations in sarcomere and cytoskeletal genes have been identified in some patients, and familial clustering is documented.

Symptoms and Clinical Presentation

Many people with LVNC are asymptomatic for years, and the condition is discovered incidentally on imaging ordered for another reason. When symptoms do appear, they typically reflect one of three problems: heart failure, arrhythmia, or thromboembolism. Common complaints include shortness of breath, fatigue, exercise intolerance, palpitations, and dizziness. Some patients present with chest pain or syncope. In advanced cases, signs of fluid retention such as ankle swelling or pulmonary congestion become prominent.

Heart Failure

LVNC can impair the pumping function of the left ventricle, leading to a reduced ejection fraction. This systolic dysfunction causes the classic symptoms of heart failure, including breathlessness on exertion and reduced exercise capacity.

Arrhythmias

The abnormal muscle architecture creates a substrate for electrical instability. Patients may experience atrial fibrillation, ventricular tachycardia, or premature ventricular complexes. Some arrhythmias can be life-threatening.

Thromboembolism

The deep trabecular recesses can stagnate blood, increasing the risk of clot formation. If a clot dislodges, it can travel to the brain, kidneys, or extremities, causing stroke or organ ischemia.

Diagnosis and Imaging

Echocardiography is usually the first test that raises suspicion. Cardiologists look for a prominent trabeculated myocardium and measure the ratio of noncompacted to compacted myocardium at end-systole. A ratio greater than 2.3:1 in adults is often cited as a diagnostic threshold, though measurement technique and operator experience affect this cut-off. Cardiac magnetic resonance imaging (MRI) provides superior tissue characterization and is considered the reference standard. It can quantify the noncompacted mass, assess fibrosis, and help distinguish LVNC from other causes of a thickened or trabeculated ventricle. Genetic testing and counseling are recommended, especially when a family history of cardiomyopathy or sudden cardiac death exists.

Risks and Complications

The main risks of LVNC are progressive heart failure, dangerous arrhythmias, stroke from intracardiac thrombi, and sudden cardiac death. The risk profile varies widely. Some patients have a relatively benign course for years, while others deteriorate quickly. Factors that worsen the prognosis include a severely reduced ejection fraction, the presence of late gadolinium enhancement on MRI (a marker of fibrosis), complex ventricular arrhythmias, and a family history of sudden death.

Management and Treatment

There is no therapy that reverses the trabeculated anatomy of LVNC. Treatment focuses on managing symptoms and preventing complications. Common strategies include:

  • Heart failure medications: ACE inhibitors, beta-blockers, and diuretics to improve symptoms and preserve function.
  • Anticoagulation: Considered for patients with a history of thromboembolism or those with severely impaired systolic function and apical trabeculations.
  • Arrhythmia management: Antiarrhythmic drugs, catheter ablation, or implantable cardioverter-defibrillators (ICDs) for patients at high risk of sudden cardiac death.
  • Lifestyle measures: Sodium restriction, weight monitoring, and avoidance of excessive exertion in symptomatic patients.

Living With LVNC

Regular follow-up with a cardiologist experienced in cardiomyopathies is essential. Monitoring typically includes periodic echocardiography or cardiac MRI, symptom review, and assessment of exercise capacity. Patients are encouraged to maintain open communication about any new palpitations, fainting episodes, or changes in breathing. A tailored exercise plan can help preserve fitness without overstraining the heart, though competitive athletics is generally discouraged when the ejection fraction is reduced.
