Diagnosis and Screening

Osteoporosis is typically diagnosed with a dual-energy X-ray absorptiometry (DXA) scan, which measures bone mineral density at the hip and spine. The result is reported as a T-score, comparing an individual's bone density to that of a healthy young adult. A T-score of -2.5 or lower indicates osteoporosis, while scores between -1.0 and -2.5 signal osteopenia, a precursor stage. The United States Preventive Services Task Force recommends screening for women aged 65 and older, and earlier for younger postmenopausal women with risk factors. Prevention and Treatment Strategies

Prevention begins in childhood and young adulthood, when building peak bone mass offers a lasting reserve. Strategies that apply across the lifespan include:

  • Adequate calcium intake, typically 1,000 to 1,200 milligrams per day for adults, through diet or supplements when needed.
  • Sufficient vitamin D, which helps the body absorb calcium, with blood levels monitored by a clinician.
  • Weight-bearing and resistance exercise, such as walking, dancing, lifting weights, or using resistance bands, which stimulate bone formation.
  • Fall prevention, including home safety modifications, balance training, and vision checks, which directly reduce fracture risk.
  • Avoiding smoking and limiting alcohol to moderate levels.

When osteoporosis is already established, treatment goes beyond lifestyle changes. Prescription medications work by either slowing bone resorption or promoting bone formation, and the choice depends on fracture risk, age, sex, and other health factors. Clinicians may use tools like FRAX, which estimates the ten-year probability of a major osteoporotic fracture, to guide decisions about who benefits most from pharmacologic therapy. Because osteoporosis management is highly individualized, anyone concerned about their bone health should discuss screening and options with a healthcare provider rather than relying on general guidance alone.
